
In the state of Michigan, individuals who have been convicted of a DUI and are looking to reinstate their driver’s license are required to participate in a restoration hearing with the Driver Assessment & Appeal Division (DAAD). A critical component of this process is the completion of a substance abuse evaluation. This evaluation serves as a formal assessment of the individual’s substance use history and current status, and it must provide compelling evidence that the individual is no longer at risk for future incidents of drunk driving.

Here is a checklist of information that you need to bring with you at the Substance Abuse Evaluations and Alcohol Assessments appointment:
- History Information
Any convictions, driving violations, and/or criminal records, with dates, blood alcohol level, history of substance use including approximate dates of previous sobriety - Treatment
Any counseling or treatment received with dates - Support Documents
AA meetings, support groups (sign-in sheets) - Letters of Sobriety
If you have reference letters completed – bring copies - 12 Panel Drug Test Results
If you have completed a drug test – bring in a copy of the results - Payment
$375.00 cash or credit card only. A $175.00 non-refundable deposit is due at the time of scheduling an appointment.
