1. Where is your office located?

Our office is conveniently located in the Image Building on 28th Street just a short distance off of the I-96 expressway. Take I-96 to the East 28th Street exit; go straight for just a few short blocks to Kraft Avenue, the office is located on the southeast corner of 28th Street and Kraft Avenue.  We are the business building located directly behind the Panera Beard restaurant on the corner of 28th Street and Kraft Avenue.

2. What types of counseling do you offer?

3.  What forms of payment do you accept?

Cash, Check, Master Card, Visa, and Discover credit and debit cards are accepted. Payment in full is due at time of visit.

4. Do you accept health insurance?

Yes, we are an in-network provider for all PPOM’s including Aetna, ASR Physicians Care, Cigna, Cofinity, ComPsych, BCBS, BCN, Humana, Magellan, Priority Health – POS, PPO, MCR, Medicaid, TriCare, United Healthcare and Value Options.

We also accept the following Employee Assistance Programs (EAP):  Life Sync, Magellan EAP,  Aetna EAP, Value Options EAP, Bensinger Dupont, Carebridge, Ceridian, Charles Nechtem EAP, ComPsych, Health Advocate EAP, MH EAP, Mutual of Omaha, Network Advantage Services, and Optum, Live & Work Well.

If you do not see your insurance listed, please call our office to see if we accept your insurance. 

5. Do you dispense medications?

No, we are not licensed to prescribe medications. You will need to contact your medical doctor or a psychiatrist for your prescriptions. Your therapist can make a recommendation to your doctor or psychiatrist.

6. Does Centennial Park Counseling offer Group Therapy or Workshops?

Yes, at the present time we offer groups for Sexual Addictions and a support group for their significant others.

7. How may we contact you for an appointment or other information?

You may call the office at (616) 949-9550 and our Administrative Assistants will be happy to assist you.  If we are unavailable, please leave a voice-mail and we will return your call ASAP, usually within the same working day Monday-Friday.
